In more extreme cases, the feces consolidate into hard masses inside the colon, called fecalomas (literally, fecal tumor ), which can require surgery to be removed. Acquired megacolon, of which there are several possible etiologies. Stercoral ulcers are sometimes observed in chronic megacolon, which may lead to perforation of the intestinal wall in approximately 3% of the cases, leading to sepsis and risk of death. Chagas is caused by Trypanosoma cruzi, a flagellate protozoan transmitted by the feces of a hematophagous insect, the assassin bug, when it feeds. People who practice colon cleansing believe that accumulations of putrefied feces line the walls of the large intestine and that these accumulations harbor parasites or pathogenic gut flora, causing nonspecific symptoms and general ill-health. Chagas can also be acquired congenitally, through blood transfusion or organ transplant, and rarely through contaminated food (for example garapa ). In the United States the Food and Drug Administration (FDA) regulates the production of equipment used in colon hydrotherapy in the USA but does not regulate their use, or the supplements used in oral colon-cleansing regimens and manufacturer claims do not require verification or supporting evidence. Some forms of colon hydrotherapy use tubes to inject water, sometimes mixed with herbs or with other liquids, into the colon via the rectum using special equipment. Colon cleansing (also known as colon therapy ) encompasses a number of alternative medical therapies claimed to remove nonspecific toxins from the colon and intestinal tract. cruzi causes the destruction, however, remains to be determined. In Central and South America, the most common incidence of chronic megacolon is that observed in about 20% of patients affected with Chagas disease. During an enema, the water is retained in the colon for approximately 15 minutes. It is probably due to an excessive production of nitric oxide, at least in ulcerative colitis.
